Academic Integrity Appeal Form
 
The regulations and guidelines governing academic integrity appeals are found in the Academic Calendar, Academic Regulation 18 (for undergraduate students) and Academic Regulation 33 (for graduate students).  Before completing this form, appellants are strongly advised to read the regulation very carefully; students are also advised to consult with SMUSA at Email.
Appeals must be filed within 10 days of the Decision Letter distribution.

A description of "Procedural Fairness" and University's rules, regulations, and policies are provided in the Calendar. If your appeal is based on compelling new evidence, please explain and attach the supporting documents.
